Conditionally reinstated licenses shall:

1. (a) Remain valid for no more than 26 weeks unless their validity is extended pursuant to Phy 404.08 or Phy 404.09; and
2. (b) Limit the practice of a physical therapist or physical therapist assistant to the supervised practice described in Phy 404.04.
