Applicants for initial licensure as physical therapists and physical therapist assistants shall be graduates of a physical therapy education program:

1. (a) Approved by CAPTE;
2. (b) Approved by APTA during the period from 1955 through 1977;
3. (c) Approved before 1955 by The Council on Medical Education and Hospitals of the American Medical Association; or
4. (d) Meeting the standards set forth in Phy 306.02 if the physical therapy education program was outside the United States and does not meet any of the standards in (a) through (c) above.
