1. (a) Applicants for initial licensure as physical therapists or physical therapist assistants shall:

   1. (1) Be of good moral character as evidenced by:

      1. a. The answers to the questions on the application form described in Phy 304.01(a);
      2. b. Any reports submitted pursuant to the requirements of the application form described in Phy 304.01(b);
      3. c. Any information about the applicant’s licenses or other authorizations to practice physical therapy; and
      4. d. The criminal history record reports required by RSA 328-F:18-a.
   2. (2) If applicable, have met the English language proficiency testing requirement of Phy 306.04;
   3. (3) Have taken and passed the national physical therapy examination for physical therapists or the national physical therapy examination for physical therapist assistants, as applicable; and
   4. (4) Have taken and passed the jurisprudence assessment module as described in Phy 303.04; and
   5. (5) Have met the continuing competency requirements as defined in Phy 302.03.
2. (b) Pursuant to RSA 332-G:7, the board shall consider military experience applicable to the education of physical therapists or physical therapist assistants when determining whether an applicant meets the educational or experience requirements for licensure.
