1. (a) An “Application for Pemit – Bulk Sterile & Non-Sterile Compounders (Including FDA Registered 503B Outsourcing Facilities)”, form Ph OF-1, revised June 2017, for a permit to license FDA registered 503B outsourcing facilities in New Hampshire may be obtained from the board or board website;

2. (b) Form Ph OF-1 shall be used for:

   1. (1) Applying for a permit to license a 503B outsourcing facility;
   2. (2) Changing the location of a currently licensed 503B outsourcing facility; and
   3. (3) Changing the ownership of a currently licensed 503B outsourcing facility.
3. (c) Form Ph OF-1 shall be filed at the board office as identified in Ph 103.03.
