1. (a) Pesticide dealers shall maintain a record of their annual pesticide sales and submit an accurate report of these sales when applying for a license renewal.

2. (b) Pesticide dealers shall report annually the following regarding pesticide sales:

   1. (1) Date of the sale;
   2. (2) Supervisory license or permit number of the person purchasing the pesticides;
   3. (3) Name of the license or permit holder as it appears on the license or permit document;
   4. (4) Quantity and container size of each pesticide purchased;
   5. (5) Brand name of the pesticide purchased; and
   6. (6) The concentration of active ingredient in the pesticide.
