The public notice and notice of hearing to the applicant and others shall contain:

1. (a) A reference to the regulatory authority, Pes 602.03, and Pes 602.06, for conduct of hearing;
2. (b) A statement of the purpose of the hearing;
3. (c) A statement of the time, date, and place of the hearing, and the manner in which comments may be submitted for consideration by the division;
4. (d) A statement of the place and time where relevant material may be examined, and the name, address, and telephone number of the person from whom further information may be obtained; and

5. (e) A description of proposed treatment(s) including:

   1. (1) A description of the area to be treated and the areas likely to be affected;
   2. (2) The name and EPA registration number of pesticides to be used;
   3. (3) The name and address of the applicant and agent; and
   4. (4) The proposed date(s) of treatment(s).
