Persons who have been issued a pesticide dealer’s license shall:

1. (a) Not display for self-service any "Prohibited-limited use" or "restricted" pesticides;
2. (b) Keep records pertaining to the acquisition, sale or disposal of "prohibited-limited use" or "restricted" pesticides and submit these records to the division on an annual basis on or before December 1, on a form provided by the division;
3. (c) Maintain and keep up to date, on a day-to-day basis, such records as are required under (b);
4. (d) Make all records pursuant to (b) available, upon request, for inspection by the division;

5. (e) Include in records being kept pursuant to (b):

   1. (1) Date of sale;
   2. (2) Supervisory registration or restricted use permit number of person making the purchase;
   3. (3) Name of registration certificate or permit holder purchasing the pesticide;
   4. (4) Quantity and container size of pesticide purchased;
   5. (5) Brand name of pesticide purchased;
   6. (6) Concentration of active ingredient in the pesticide;
   7. (7) Name of individual preparing the record; and
   8. (8) Name of dealership.
