1. (a) The procedures set forth in this section shall apply to appeals to the board from a decision of the division:

   1. (1) To deny an application for a permit or certificate of registration;
   2. (2) To revoke, suspend, or modify a permit or certificate of registration;
   3. (3) To impose an administrative fine;
   4. (4) To issue an order; or
   5. (5) To issue or deny a special permit.
