1. (a) The single-use pier permit application form shall require the division director or an employee of the division to enter the following information provided by an applicant under Pda 606.03(b):

   1. (1) The applicant’s full legal name;
   2. (2) The applicant’s permanent address;

   3. (3) The applicant’s telephone number(s) including:

      1. a. Business telephone number;
      2. b. Home telephone number;
      3. c. Cell telephone number, if different from permanent telephone number; and
      4. d. An emergency contact telephone number;

   4. (4) The following information pertaining to the vessel and registration and identification numbers:

      1. a. Vessel name;
      2. b. New Hampshire or other state registration number, or federal documentation number, as applicable;
      3. c. The identification number(s) for any applicable fishing permits held by the applicant;
      4. d. Vessel LOA;
      5. e. Vessel width;
      6. f. Vessel draft;
      7. g. Vessel color; and
      8. h. Type of vessel; and
   5. (5) The reason(s) the applicant wishes to use the pier.
2. (b) The form shall require the division director or an employee of the division to verify that the applicant has displayed the documentation required under Pda 606.03(d)(1).

3. (c) By their signature, the applicant shall certify the following:

   “I certify that the statements and information in this application are to the best of my knowledge and belief true, accurate and complete. I am aware that my pier use permit may be withdrawn by the Pease Development Authority for submitting false statements or information or omitting required statements or information.”

4. (d) The applicant shall sign and date the application.
