Any person seeking an appointment as a temporary pilot shall:

1. (a) Submit to the division a completed application form, as described in Pda 306.03, including all required certifications; and
2. (b) Meet the requirements set forth in Pda 305.06(b)(1), (2), (4), (6), and (7).
