1. (a) Date;
2. (b) Name of person to whom the order is directed;
3. (c) Order to shutdown tramway;
4. (d) Tramway plate number;
5. (e) Statement of duration of the order;
6. (f) Signature of board member issuing the order, or his or her agent; and

7. (g) Affidavit of service, including:

   1. (1) Name of person to whom the order is served;
   2. (2) Date and time of service; and

   3. (3) Signature of person making service.
