(a) To register the new construction, relocation, altered in length, or modification and registration of T-bars, J-bars, platterpulls, chairlifts, aerial tramways or gondolas, the following shall be submitted to the board with the form Pas 5 “Tramway Application / Registration Certificate”, revised 1/2022:

1. (1) Certificate from designer with original signatures and drawings list with revision dates and notes of changes made, not shown on the listed drawings submitted to the board;
2. (2) Construction certificate with a drawing list as built and original signatures submitted to the board within 30 days of the acceptance test;
3. (3) Completed acceptance test and inspection;
4. (4) Completed written evacuation plan and documentation of training;

5. (5) Rope inspection reports submitted to the board for the following:

   1. a. Haul;
   2. b. Counterweight; and
   3. c. Other wire rope or strand;
6. (6) On-site operations manual;
7. (7) On-site maintenance manual, and documentation of all required maintenance; and
8. (8) Documentation that any variances required have been granted.
