(a) To register the new construction, relocation, altered in length, or modification of conveyors or carousels”, the following shall be submitted to the board along with the form Pas 5 “Tramway Application / Registration Certificate”, revised 1/2022:
- (1) Certificate from designer with original signatures and drawing list submitted to the board within 30 days of the acceptance test;
- (2) Completed acceptance test and inspection;
- (3) Onsite operations manual;
- (4) On-site maintenance manual; and
- (5) Documentation that any variances required have been granted.
- (b) The operator shall sign the completed Pas 5 form under penalty of unsworn falsification pursuant to RSA 641:3.
