1. (a) To register the new construction, relocation, altered in length, or modification of fiber rope tows or wire rope tows, the following shall be submitted to the board along with the form Pas 5 “Tramway Application / Registration Certificate”, revised 1/2022:

   1. (1) Certificate from designer with original signatures and drawings submitted to the board within 30 days of the acceptance test;
   2. (2) Completed acceptance test and inspection;

   3. (3) Wire rope test certificates submitted to the board for the following:

      1. a. Haul rope;
      2. b. Tensioning rope; and
      3. c. Stay ropes;
   4. (4) On-site operations manual;
   5. (5) On-site maintenance manual; and
   6. (6) Documentation that any variances required have been granted.
2. (b) The operator shall sign the completed Pas 5 form under penalty of unsworn falsification pursuant to RSA 641:3.
