- (a) The owner or operator of any tramway shall submit a Pas 6 “Tramway Incident Report” form, revised 1/2022, to the board of all incidents, including all those requiring an oral report, involving any lift system failure affecting the safety or integrity of any tramway, or resulting in bodily injury on a tramway, including loading and off-loading areas.
- (b) The report shall be signed by the area operator or owner and sent to the board within 5 days of the date of the incident.
