1. (a) Before beginning work in occupational therapy, an occupational therapy assistant shall:

   1. (1) Give to the person intending to provide supervision to the assistant a copy of Occ 400 and the supervision form described in Occ 407.10;
   2. (2) Discuss the supervision requirements with the person intending to provide supervision; and
   3. (3) Submit to the OPLC the completed supervision form.

2. (b) An occupational therapy assistant shall submit a revised or additional supervision form to the OPLC:

   1. (1) Within 30 days of the date of change each time there is a change in the person providing supervision to the assistant; and
   2. (2) Whenever the occupational therapy assistant begins work for any other employer as an occupational therapy assistant.
