1. (a) A licensee wishing to renew their current license shall complete and submit the following:

   1. (1) A “Universal Application for License Renewal” as required in Plc 308.05, that provides the information required by Plc 308.06, the additional information and documentation required by Plc 308.07, and is signed and attested to as required by Plc 308.08;
   2. (2) Occupational therapy assistants who are employed occupational therapy assistants at the time of submitting their application for license renewal shall print their name and indicate on the “Addendum to the Universal Application for License Renewal for Occupational Therapy Assistants” revised 1/2026, whether they have submitted complete supervision form(s) as required by Occ 406.10, that are signed and dated; and
   3. (3) The application fees identified in Plc 1002.35.
2. (b) Pursuant to RSA 310:8, III, licenses to be renewed shall lapse if the renewal applicants do not submit the completed renewal application, the application related fees for renewal, and any supporting documents required for renewal on or before the license expiration date.
3. (c) Renewal applicants whose licenses have expired shall not practice occupational therapy until their licenses have been reinstated by the office of professional licensure and certification (OPLC).
