Applicants for initial licensure as an occupational therapist or occupational therapy assistant shall confirm on the “Addendum to the Universal Application for Initial License for Occupational Therapy”, revised January 2026 that the following documents supporting their applications have been submitted:

1. (a) Official transcripts showing all relevant education as described in RSA 326-C:5(c) by:

   1. (1) The issuing institution(s) directly to the OPLC; or
   2. (2) The applicant in one or more envelope(s) sealed by the institution(s) in such a manner that it would be evident to the OPLC if the envelope(s) had been opened;
2. (b) Verification that the applicant passed the examination sent by NBCOT directly to the OPLC; and
3. (c) A criminal history records check as required pursuant to RSA 328-F:18-a.
