In addition to the information required by the application for initial licensure required by Occ 302.01, the applicant for occupational therapist or occupational therapy assistant licensure shall answer “yes” or “no” to the question “Have you completed the supervised field work experience as required by RSA 326-C:5, III?” on the “Addendum to the Universal Application for Initial License for Occupational Therapy”, revised January 2026.
