1. (a) Applicants who wish to apply for initial licensure as an occupational therapist or occupational therapy assistant shall do so by submitting to the office of professional licensure and certification (OPLC) the following:

   1. (1) The completed “Universal Application for Initial License” required by Plc 304.01;
   2. (2) The required information specified by Plc 304.03;
   3. (3) The required information and documentation required in Plc 304.04; and
   4. (4) The initial license fee required by Plc 1002.35.
2. (b) Applications shall be reviewed by the OPLC as required by Plc 304.06 and Plc 304.08.

3. (c) Any applicant wishing to challenge the denial of an application for initial licensure shall

   submit a written request for a rehearing to the OPLC within 30 days of notification of denial pursuant to Plc 206.31.
