N.H. Code Admin. R. Nur 704.05

Coordinator Responsibilities

(a) The coordinator shall:
1. (1) Be responsible for planning, implementing and evaluating the program;
2. (2) Maintain:
  1. a. Course documents, including descriptions of each course’s content;
  2. b. A list of nursing-related activities;
  3. c. A list of individuals who successfully complete the program; and
  4. d. A list of the as dates of all programs;
3. (3) Maintain a physical presence in New Hampshire during all hours of instruction in the nursing assistant program;
4. (4) Provide a certificate to the student upon successful program completion; and
5. (5) Submit to the board names, address, telephone number and employer, if known, of those persons successfully completing a nursing assistant program within 30 days of program completion.
