Documentation of Legal Right to Work

(a) Proof of the legal right to work in the United States shall be required.
(b) The applicant shall provide one of the following:
1. (1) A certified copy of birth certificate;
2. (2) A copy of a certificate of naturalization;
3. (3) A copy of a legal permanent resident card; or
4. (4) A copy of a valid United States passport.
