1. (a) All parties seeking a waiver as authorized by RSA 217‑A:10, III, shall apply for a permit pursuant to (b)-(d) below.

2. (b) All applicants shall provide the following information:

   1. (1) Applicant's name and address;
   2. (2) Date of application;
   3. (3) Applicant's affiliation, such as business, college or organization;
   4. (4) If applicant is a student, name of faculty advisor;
   5. (5) Purpose of permit request;
   6. (6) Duration of study;
   7. (7) Location of study area; and
   8. (8) Signature of applicant.
3. (c) All information shall be legible.

4. (d) The application shall be emailed, mailed or hand-delivered to:
