1. (a) The board shall reinstate the licenses of individuals whose licenses were refused renewal pursuant to Nat 402.04(a)(4) if such individuals:

   1. (1) Are not subject to license denial pursuant to Nat 402.04(a)(1), (a)(2) or a)(3);
   2. (2) Complete their outstanding continuing education obligation(s) within the applicable time period set forth in (b) below;
   3. (3) Meet the application requirements set forth in (c) below; and
   4. (4) Do not practice naturopathic medicine until the board has reinstated their licenses.

2. (b) Outstanding continuing education obligations(s) shall be completed:

   1. (1) Within 180 days of the board's refusal to renew licensure; or

   2. (2) After such deadline if the reinstatement applicant was unable both to practice naturopathic medicine and to complete his or her continuing education for at least 180 days as a result of:

      1. a. His or her disability, disease, or severe illness;
      2. b. The death, disability, disease, or severe illness of the reinstatement applicant's natural or adoptive mother, father, sister or brother;
      3. c. The death, disability, disease, or severe illness of the reinstatement applicant's guardian or foster parent;
      4. d. The death, disability, disease, or severe illness of the reinstatement applicant's legal ward or foster child; or
      5. e. Other circumstances beyond his or her control which prevented him or her from practicing naturopathic medicine and meeting outstanding continuing education requirements.

3. (c) Reinstatement applicants shall submit to the board a letter that:

   1. (1) Requests licensure reinstatement;

   2. (2) States that his or her outstanding continuing education obligations were completed:

      1. a. Within 180 days of the board's refusal to renew licensure; or
      2. b. Later than 180 days of the board's refusal to renew licensure because of one of the reasons listed in (b)(2) above; and
   3. (3) Contains documentation in accordance with Nat 402.07(d)(1) and (2) of completion of the required continuing education.
