Persons who are not currently licensed as a doctor of naturopathic medicine or naturopathic physician in any state, the District of Columbia, or U.S. territory and who wish to apply for a license as a doctor of naturopathic medicine shall do so by submitting an application which contains:

1. (a) The completed application form described in Nat 302.03;
2. (b) The supporting materials specified by Nat 302.04; and
3. (c) The license fee specified by Nat 307.01(a).
