Any interested person may, before 30 days after final adoption of a rule, request a written explanation of that rule by making a written request to the board and including in the request:

1. (a) The name and address of the person making the request; or
2. (b) If the request is that of an entity, the name and address of the entity and the name and address of the representative authorized by the entity to make the request.
