Identification of Parties

(a) At the opening of a hearing, the board shall:
1. (1) Determine the identities of all parties and their representatives and witnesses;
2. (2) Obtain their addresses and the correct spelling and pronunciation of their names and addresses;
3. (3) Record on the report of hearing, the name of the respondent and his representative(s), and all witnesses appearing; and
4. (4) Identify themselves, their official titles and functions and briefly summarize the following:
  1. a. The purpose of the hearing;
  2. b. The scope of the hearing and the issues involved;
  3. c. The procedural rights of all parties; and
  4. d. The statute(s) and/or rule(s) under which the hearing is being held.
