Documentation of public professional presentations relating to midwifery shall include:

1. (a) A copy of the official program of the presentation; or

2. (b) Written verification signed by a representative of the program's sponsor showing:

   1. (1) The title of the presentation;
   2. (2) The name of the midwife as presenter;
   3. (3) The date of the presentation;
   4. (4) The hours during which the presentation took place; and
   5. (5) The type of audience attending the presentation.
