1. (a) Documentation of informal independent study shall include:

   1. (1) The applicant's signed written statement setting forth, for each item read or listened to, or videotape or digital video disc viewed:

      1. a. The title of the item read or viewed;
      2. b. The author and publisher of the item read or the producer of the item viewed;
      3. c. The clock hours spent in reading or viewing; and
      4. d. The date of completion of the reading or viewing; and
   2. (2) The applicant's one-page, double-spaced report, in 12 point Times New Roman font, of the major thesis or theses of the item read or viewed describing how it applies to midwifery.
2. (b) The effect of the signature required by (a)(1) above shall be to assert the accuracy of the information provided.
