For midwives not eligible for recertification under Mid 403.04 because they do not meet the recency of practice requirements of Mid 403.04(b), the midwifery council shall reinstate a certificate if the applicant for recertification:

1. (a) Has been provisionally certified pursuant to Mid 403.07; and
2. (b) Within 78 weeks of having been provisionally certified has met the supervised midwifery practice requirements set forth in Mid 403.08 and Mid 403.09.
