The midwifery council shall reinstate a certificate expired no more than 90 days if:

1. (a) The failure to apply timely and completely for renewal of recertification resulted from one of the following circumstances of hardship:

   1. (1) Illness or injury sufficiently serious to prevent the midwife from engaging in his or her usual work for at least 2 consecutive weeks;
   2. (2) Illness in the midwife's household or in the midwife's immediate family sufficiently serious to prevent the midwife from engaging in his or her usual work for at least 2 consecutive weeks;
   3. (3) Death in the midwife's household or of a member of her immediate family;
   4. (4) Active military duty; or

   5. (5) The destruction by fire, flood, or other accidental or natural disaster of:

      1. a. Continuing education records; or
      2. b. Other materials required by Mid 402.07 to be submitted with a renewal application;
2. (b) The midwife is eligible under Mid 402.02 for renewal of certification; and

3. (c) The midwife, within 90 days immediately following the expiration of the certificate:

   1. (1) Completes the renewal process set forth in Mid 402.03; and
   2. (2) Makes a written request for recertification based on one of the reasons set forth in (a) above.
