Any midwife wishing to renew a certificate shall submit the following materials to the midwifery council no later than 30 days before the date of expiration shown on the certificate being renewed:

1. (a) The completed renewal application form provided by the midwifery council;
2. (b) The supporting documentation described in Mid 402.07; and
3. (c) The renewal fee specified by Mid 306.03.
