Business applicants for licensure shall provide the following documents supporting their applications:
- (a) Proof of good standing with the secretary of state;
- (b) Poof of liability insurance; and
(c) A List of all current licensed employees that includes:
- (1) Full name as listed on their license; and
- (2) N.H. license number.
