In the event a complaint petition is considered a contested case which cannot be settled under Mavs 204.02, and a hearing is scheduled, all parties shall be notified of the:

1. (a) Time, place, and nature of the hearing;
2. (b) Statutory authority under which the hearing is to be held;
3. (c) Particular sections of the statutes or rules involved;
4. (d) Issues involved;
5. (e) Right to have an attorney present to represent the party at the party’s expense; and
6. (f) Right to have the department provide a certified shorthand court reporter at the party’s expense.
