Applicants for a sports entertainment complex license shall complete and submit Form LTW-43, “License Type Worksheet-Sports Entertainment Complex” (revised 8/2020) providing all required information, acknowledgments, and attach a copy of the applicant’s contract for operating under this license if the applicant is not the owner of the complex.
