Applicants for a tobacco retailer, tobacco sampler or a tobacco vending license shall complete and submit Form LTW-17, “License Type Worksheet-Retail Tobacco / Tobacco Sampler / Tobacco Vending” (revised 8/2020) providing all required information and acknowledgments.
