Applicants for a beer festival license shall complete and submit Form LTW-14, “License Type Worksheet-Beer Festival” (revised 8/2020) providing all required information, acknowledgments, and attach the following:

1. (a) Proof of non-profit status from the New Hampshire Secretary of State; and
2. (b) A letter of designation from a non-profit group.
