Applicants for a sports recreation facility license shall complete and submit Form LTW-05, “License Type Worksheet-Sports Recreation Facility” (revised 2/2022) providing all required information, acknowledgments, and attach a business plan of operations which shall detail the type of sports/recreation provided by the applicant and the physical facility in which their operations are carried out.
