1. (a) The certified vocational rehabilitation provider (CVRP) shall meet the definition required for one of the categories below:

   1. (1) Current certified rehabilitation counselor (CRC);
   2. (2) Current certified disability management specialist (CDMS); or
   3. (3) Current certified vocational evaluator (CVE).

2. (b) The certified vocational rehabilitation provider intern (CVRPI) shall meet all of the following criteria:

   1. (1) The individual shall submit a written professional development plan (PDP) to the commissioner which outlines how one of the categories listed in (a) above shall be achieved within 5 years from the date of application;
   2. (2) The individual shall be supervised by a certified vocational rehabilitation provider (CVRP); and
   3. (3) The individual shall file annually with the commissioner a progress report on the anniversary date of application regarding the status of the written professional development plan (PDP).
