1. (a) Notice shall be provided to policyholders at least 30 days prior to the date of discontinuance and include:

   1. (1) The policy number and coverage type;
   2. (2) The date of discontinuance of such policy; and
   3. (3) Company contact information, including a toll-free telephone number.
