No policy of individual accident and health travel insurance shall be issued or delivered to any person or entity in this state, nor shall any application, rider or endorsement be used in connection therewith until a copy of the policy form, description of coverage form and premium rates, have been filed with and approved by the commissioner pursuant to RSA 415:1.
