Each participating dental provider shall:

1. (a) Hold an active license to practice dentistry in the state in which he or she practices;
2. (b) Be a New Hampshire enrolled Title XIX provider; and
3. (c) Request and obtain prior authorization from the department, in accordance with He-W 566.07.
