Covered HVNH and child/family health care support services shall include the following services delivered in accordance with a plan of care as described in He-W 549.07:

1. (a) HVNH services which shall:

   1. (1) Focus on healthy birth outcomes;
   2. (2) Include initial assessment, not to exceed 2 visits, and subsequent visits;

   3. (3) Be conducted through face-to-face visits at:

      1. a. The pregnant woman’s or child’s home;
      2. b. The participating provider or health care provider agency; or
      3. c. Any other location, as documented in the pregnant woman’s or child’s progress notes; and

   4. (4) Include the following:

      1. a. Prenatal assessment, support and education including, but not limited to:

         1. 1. Nutrition assessment and education;
         2. 2. Networking with social support services;
         3. 3. Accessing and receiving continuous prenatal care; and
         4. 4. Health implications of risk behavior, such as smoking;
      2. b. Parenting skills and child development education;
      3. c. Parental assessment, support and education to encourage parental coping and the parent’s ability to develop attachment and nurturing skills;
      4. d. Family planning;
      5. e. Community support guidance for the purpose of securing and maintaining a safe and healthy home;
      6. f. Education and support to assure recipient has access to, and obtains, preventive and acute health care; and
      7. g. Follow-up activities related to any of the above services; and

2. (b) Child/family health care support services which:

   1. (1) Shall include initial assessment and subsequent visits;
   2. (2) Shall be family-centered and child-focused;

   3. (3) Shall be conducted through a face-to-face visit with the recipient’s parent or guardian at:

      1. a. The recipient’s home;
      2. b. The participating provider or health care provider agency; or
      3. c. Any other location, as documented in the recipient’s progress notes;
   4. (4) May be conducted on a limited basis by telephone or via a video conference in lieu of a face-to-face visit if the service can be provided over the phone or via a video conference and provision of the service over the phone or via a video conference would not compromise the intent or anticipated result of the service; and

   5. (5) Shall include any of the following:

      1. a. Nutrition assessment and education;
      2. b. Family support activities geared towards developing and maintaining family support systems, including, but not limited to, education and consultation;
      3. c. Child-focused, family-centered education to a recipient’s parents or guardians in the areas of physical and behavioral health, social support, parenting education, and household and environmental safety;
      4. d. Child development screening, education, and guidance;
      5. e. Community support guidance for the purpose of securing and maintaining a safe and healthy home;
      6. f. Parental assessment, support and education to encourage parental coping and the parent’s ability to develop attachment and nurturing skills;
      7. g. Education and support to assure recipient has access to, and obtains, preventive and acute health care; and
      8. h. Follow-up activities related to any of the above services.
