PDN shall be a covered service when:

1. (a) It is part of the recipient’s medical regimen and rendered under the order and general direction of the recipient’s physician or other licensed practitioner;

2. (b) It is provided in one of the following locations:

   1. (1) The recipient’s home; or
   2. (2) In locations other than the recipient’s home when routine life activities take the recipient outside of the home if the services would have otherwise been provided in the recipient’s home; and
3. (c) Prior authorization in accordance with He-W 540.07 has been requested and obtained.
