1. (a) Cost effectiveness shall be redetermined annually concurrent with the policyholder’s annual open enrollment in the employer group health plan, or any time there is a change in the group health plan.
2. (b) It shall be the responsibility of the policyholder to submit the current employer group health plan, as described in He-W 511.04(e) and (f) above, when employer open enrollment starts, there is a change in the employer group health plan, or there is a change in the family status on the employer group health plan.
