Provider Participation

Effective Jul 1, 2013#10357, eff 7-1-13Former Division of Human Services

All participating family planning providers shall be:

(a) Licensed by the state in which s/he practices or be a NH certified midwife; and
(b) A New Hampshire enrolled Title XIX provider.
