1. (a) The ICF/IID shall establish an interdisciplinary quality improvement committee which:

   1. (1) Shall have a minimum of 3 members, including the medical director, an individual representing nursing, and an individual representing administration;
   2. (2) Shall meet at least quarterly to evaluate quality improvement activities; and
   3. (3) Shall make recommendations to the administrator to improve the quality of care.

2. (b) The quality improvement committee shall be responsible for:

   1. (1) Determining the information to be monitored;
   2. (2) Determining the frequency with which information will be reviewed;
   3. (3) Determining the indicators that will apply to the information being monitored;
   4. (4) Evaluating the information that is gathered;
   5. (5) Determining the action that is necessary to correct identified problems;
   6. (6) Recommending corrective actions to the licensee; and
   7. (7) Evaluating the effectiveness of the corrective actions and determine additional corrective actions as applicable.
