When an AFCR no longer intends to operate, an oversight agency shall notify the department in writing of the following:

1. (a) The name of the AFCR;
2. (b) The certificate number of the AFCR;
3. (c) The address of the AFCR;
4. (d) The date the AFCR closed or will close; and
5. (e) The location that the resident(s) have moved to, including the name of the home and certificate number of the home, if available.
