Quality assurance for a conventional or virtual simulator shall include:

1. (a) Acceptance testing;
2. (b) Periodic verification of system performance;
3. (c) Be performed in accordance with “Comprehensive QA for Radiation Oncology,” (Updated by Table II)(1994) prepared by American Association of Physicists in Medicine (AAPM), Radiation Therapy Committee Task Group No. 40; or
4. (d) Be performed in accordance with “Quality Assurance for Computed –Tomography Simulators and the Computed Tomography-Simulation Process” (2003) prepared by AAPM Radiation Therapy Committee Task Group 66 as incorporated by reference and included in Appendix A.
