1. (a) A licensee shall provide oral and written radiation safety instruction for all personnel caring for patients or human research subjects who cannot be released under He-P 4035.25.
2. (b) Refresher training shall be provided at intervals not to exceed 1 year.

3. (c) To satisfy the requirements of He‑P 4035.36(a), the instruction shall describe the licensee’s procedures for:

   1. (1) Patient or human research subject control;

   2. (2) Visitor control, including:

      1. a. Routine visitation to hospitalized individuals in accordance with He-P 4020.13(a)(1); and
      2. b. Visitation authorized in accordance with He-P 4020.13(c);
   3. (3) Contamination control;
   4. (4) Waste control;
   5. (5) Notification of the radiation safety officer, or his or her designee, and an authorized user in case of the patient’s or human research subject’s death or medical emergency; and
   6. (6) Training, which shall be commensurate with the duties of the personnel, as required by He‑P 4019.

4. (d) A licensee shall keep a record of:

   1. (1) Individuals receiving operational and safety instructions required by He‑P 4035.36(a);
   2. (2) A description of the instruction;
   3. (3) The date of instruction; and
   4. (4) The name(s) of the individual(s) who gave the instruction.
5. (e) The record required in He‑P 4035.36(d) shall be maintained for inspection by DHHS/RHS for 3 years.
